Value of recording duration of post-traumatic amnesia

This chapter covers important clinical information pertaining to traumatic brain injury (TBI): prognosis, outcome measures, and prevention. Providing an individualized prognosis is one of the most challenging aspects of TBI management. Successful prediction of survival in more severe TBI has improved; however, this success does not translate to prediction of detailed functional outcome for less severe cases of TBI. A number of outcome measures have been developed for assisting with predicting and monitoring recovery, and these will be reviewed in this chapter. Finally, a discussion pertaining to preventative measures will be presented.

La durée d’amnésie post-traumatique (APT), phase de vigilance altérée fréquente après traumatisme crânien (TC), a fait preuve de son utilité en tant qu’indice de sévérité du TC et prédicteur du devenir post-TC. Or, il n’existe pas d’outil standardisé pour l’évaluer chez l’enfant traumatisé crânien français.

Traduire et étalonner le Children’s Orientation and Amnesia Test (COAT), inspiré du Galveston Orientation and Amnesia Test (GOAT) qui estime prospectivement la durée d’APT chez l’adulte.

Mise au point d’une version française du COAT (COAT-VF) comportant 16 items : sept d’orientation générale, cinq d’orientation temporelle et quatre de mémoire. Des données normatives ont été recueillies auprès de 137 enfants âgés de quatre, six, huit et dix ans, scolarisés pour moitié en Réseau d’éducation prioritaire (REP) et pour moitié en dehors.

Les performances sont sensibles au niveau socioculturel de l’enfant et s’améliorent avec l’âge. Il n’y a pas d’effet du sexe sur les résultats.

Le COAT-VF, dont la passation est simple et courte, rend possible une évaluation plus précise de la longueur de l’APT chez l’enfant français.

The duration of post-traumatic amnesia (PTA), the period of altered memory that often follows traumatic brain injury (TBI), is a useful index of severity of TBI and a predictor of outcome after TBI. Yet, evaluating PTA in French-speaking brain-injured children is not standardized.

We aimed to translate into the French language and standardize the Children’s Orientation and Amnesia Test (COAT), inspired by the Galveston Orientation and Amnesia Test (GOAT), which estimates PTA duration in adults.

Development of a French version of the COAT (COAT-VF) with 16 items: 7 of general orientation, 5 temporal orientation and 4 memory. Normative data were collected for 137 children aged 4, 6, 8 and 10 years who were attending school.

Performance on the COAT-VF was influenced by age and sociocultural factors but not sex.

The COAT-VF, short and simple to administer, allows for an accurate evaluation of PTA duration in French children.
