↵1-150 Peripheral vascular disease was defined as the absence of both foot pulses or femoral bruits, a history of intermittent claudication, or a history of peripheral vascular surgery.
↵1-151 Ischaemic heart disease included either angina pectoris or previous myocardial infarction.

Diabetes mellitus was defined as history of diabetes mellitus at the time of assessment, on diet, oral hypoglycaemic drugs or insulin.
