Analysis of 2000 consecutive UK tonsillectomy specimens for disease-related prion protein

Summary

Variant Creutzfeldt-Jakob disease (CJD) is thought to be caused by dietary or other exposure to bovine spongiform encephalopathy (BSE) prions. The prevalence of preclinical or subclinical prion infection in the UK is currently unknown. Since clinical variant CJD is uniformly associated with tonsillar prion infection, we screened 2000 anonymous surgical tonsillectomy specimens for disease-associated prion protein. Analysis by both high sensitivity immunoblotting and immunohistochemistry detected no positive cases. However, this negative result cannot provide reassurance that relevant community infection is unlikely because of the fairly small sample size, demographic and age-related factors, and unknown test sensitivity during the prolonged incubation period. Nevertheless, our findings establish a protocol for prevalence screening on a national scale.

    Mathematical modeling suggests a maximum likelihood of 3000 infected people (95% confidence interval (CI) 520–6810) in the United Kingdom, mainly in the 10–30-year age group (Clarke and Ghani, 2005). The data generated by study of tonsil and appendectomy samples have also been revealing (Ironside et al., 2000; Hilton et al., 2002; Frosh et al., 2004). The first of the studies revealed three out of 12,674 samples positive for abnormal prion protein (PrP) on Western blot (Hilton et al., 2004a) where the specificity is thought to be high but the sensitivity uncertain (Hilton et al., 2004b).

    The global estimated prevalence was 493 per million (95% confidence interval 282–801) or 1 per 2000 (95% confidence interval 1/3500–1/1250) (Gill et al., 2013). The studies on frozen or paraffin-embedded tonsil tissues did not bring additional useful data for the estimation of prevalence of healthy carriers in the United Kingdom (Frosh et al., 2004; Clewley et al., 2009; de Marco et al., 2010). The genotype at codon 129 of the PRNP gene could be determined for 2 out of 3 patients of the first study (all valine-valine) and for all the patients of the second study (8 methionine-methionine, 4 methionine-valine, and 4 valine-valine).
