I had a 14 year old lady who presented with bilateral lateral rectus weakness, right more than left, along with stiff person syndrome, proven by antibodies to glutamic acid decarboxylase. With intravenous immunoglobulins and steroids, her ophthamoplegia resolved and stiff person syndrome improved. She had no evidence of myasthenia gravis, clinically or electrophysiologically. Tensilon test was negative...
