An advanced diffusion MRI shows new aspects of the normal appearance cortical abnormalities in patients with multiple sclerosis (MS).

Preziosa et al report on microstructural abnormalities observed in MS1 and specifically describe normal-appearing cortex and cortical lesions assessed in vivo using neurite orientation dispersion and density imaging (NODDI), an advanced diffusion MRI technique. The study included a large cohort of 172 patients with MS (101 relapsing-remitting, 71 progressive) and 62 healthy controls. They found that normal-appearing cortex in MS was characterised by a decreased intracellular volume fraction (ICV_f), which is more evident in cortical lesions.
