Symptoms improved after vitamin B12 replacement therapy. Insulin tolerance testing showed that the patient lacked catecholamine, heart rate, and sweating responses to hypoglycaemia. This indicates that pernicious anaemia may cause orthostatic hypotension owing to failure of noradrenaline release.
